Insurance: We participate with major insurance
companies such as Medicare, Blue Cross/Blue Shield
of Nebraska, Midlands Choice and Medicaid. Please
contact us regarding your insurance. Our billing
specialist will be happy to assist you in obtaining
your insurance benefits, any preauthorization that
your insurance may require and co-payment
information prior to your first appointment. All
insurance claim forms will be prepared in our office
and sent directly to your insurance company for
payment. Please have your identification card
available when calling.
